58-Year-Old Female Undertaker Crushed to Death by Hoist Used to Move Bodies While Working Alone

Sally Blundell, a 58-year-old funeral director, was crushed to death by a machine used to move coffins on Dec.1, 2023 at the East of England Co-op Funeral Services branch in Swaffham, Norfolk. A colleague from a different branch found her body after a bereaved family couldn't find any staff members in the branch. The Health and Safety Executive examined the 441 lb hydraulic scissor lift mortuary trolley in 2018 and found that the return spring on the control handle was not fully operating and didn't close the operating valve without manual assistance. The coroner's cause of death was "contusion and compression of the chest by an external object."
