The Office of the United States (OIG) has revealed that U.S. General Aguto had twice improperly given control of unsecured maps to his staff while traveling by train, leading to the loss of the maps for more than 24 hours in early April 2024. The Ukrainian security worker found the maps on the train and returned them to the U.K. Embassy in Kyiv after Aguto's staff launched a search. The OIG interviewed 33 witnesses and Aguto himself, who claimed he frequently used the maps to brief officials on the status of Ukraine. He also stated that he had received verbal permission to consume alcohol while on official business. The day after the dinner, Aguto fell twice in his hotel room and fell out three times, including a third time while arriving at the US Embassy.
