Indiana audit of Medicaid attendant care program flags widespread violations

A sweeping audit by the Indiana Family and Social Services Administration has found nearly $200 million in improper payments to home- and community-based attendant care services. The auditors reviewed 625 claims across five high-risk providers between January 2022 and March 2025. The worst issue was a lack of criminal background checks, which resulted in unscreened individuals being sent directly into homes of older adults and people with disabilities. The Department of Family and Safety is expanding oversight to include prepayment reviews for repeat offenders and stronger electronic visit verification checks.
