AI Now Decides if Texas Medicare Patients Receive Certain Types of Care • The Austin Chronicle

The Wasteful and Inappropriate Service Reduction (WISeR) Model, a federal program run by private AI company, has been implemented in Texas by Dr. Mehmet Oz and the Centers for Medicare & Medicaid Services (CMS) in June 2025. The program, which allows Texas Medicare patients to receive certain types of care, requires patients to pass an AI-powered checkpoint to obtain prior authorization for procedures such as skin graft, nerve stimulator, or skin graft. This is one of the largest expansions of prior authorization in the 60-year history of traditional Medicare. Despite this, Dr. Jack Resneck, former president of the American Medical Association, noted that prior authorization often involves a significant amount of paperwork and often delays medically necessary care. The AMA has dispelled the WISER requirement, stating that 93 percent of physicians reported that PA causes care delays, 82 percent indicated that process can lead to treatment abandonment, and 29 percent said it had led to a serious adverse event (hospitalization, disability, or death) for patients in their care.
