Cleveland Clinic at-home hospital-level care launches in Ohio

The Cleveland Clinic has launched a new at-home care program, Hospital Care at Home, in Ohio. The program allows patients to receive hospital-level care while sleeping in their home. Patients will receive virtual monitoring by caregivers based in Florida, as well as in-person medical visits. To enroll, patients must have a qualifying condition, live within 25 miles of Cleveland Clinic Fairview or Avon hospitals and pass a medical evaluation. The cost of the program is the same as the cost of a hospital stay, and it is covered by Medicare, Medicare Advantage, and many commercial insurance plans. While the program has been tested in Florida since 2023, it has helped over 4,000 patients and reduced readmissions. It also resulted in 60% fewer patients needing to go to a skilled nursing or rehabilitation facility after discharge.
